Basic Life Insurance/Accidental Death & Dismemberment

Term life insurance coverage will be provided for the employee at no cost. This term insurance provides a gross benefit equal to one times your annual base salary. Accidental deaths are covered at two times your annual base salary. Please refer to your Certificate of Insurance for the Benefit Schedule on all insurance, including dismemberment benefits. Appropriate taxation may apply. New employees become eligible for coverage following a 90-day wait period.
