EIS Procedures
401(k) Signup
After completing one year of service, EIS employees are eligible to
signup for the EIS 401(k) retirement plan. Follow these steps to signup:
Review the investment options in the 401(k) folder (available from Peter or
Marc). Determine which investments you would like and how, by percentage, you
would like your deferrals split between them.
Determine the deferral percentage you would like taken from your paycheck
each month (0% to 12%).
Determine your beneficiaries.
Print & complete the 401(k) Signup Forms below.
